Things to bring with you to your appointment:

 Your Current Insurance Card(s)

  • Your Co-pay: (We accept cash, check, money order and
    Visa, Master Card or Discover cards).
  • Completed Patient Paperwork (*PLEASE DO NOT PRINT ON DOUBLE SIDE. Each form has a different place in the chart and you will be asked to fill out some of the forms at the office again. Thank you ):

Insurance

Insurance policies and guidelines are always changing and vary from plan to plan. Before your visit, please check with your insurance company to be sure:

  • •Dr. Collins is listed with your plan as a provider
  • •Your plan covers the services you need
  • •If needed, you have the necessary referral from your primary care physician

*Please arrive 10 minutes prior to your appointment with your completed paperwork. If your paperwork is not complete, please arrive 20 minutes early to allow yourself time to finish so as not to delay your appointment.

Canceling or rescheduling your appointment:

If for any reason you are unable to keep your appointment and need to reschedule, please notify us. We appreciate 24 hours notice. We understand that unexpected things come up.

If you do not show up for your appointment and did not call beforehand to cancel —after two No Show appointments, we will charge your account $50.

Medical Records

A Records Release form is necessary for release of your medical records from our office to another doctor's office, a hospital or to you. Please download, complete, sign, date and return to us by fax or mail.

If you are being referred by another provider to our office it is important that your records from their office be available for the provider to review before your scheduled visit. Please allow at least a couple of days to get them from the referring doctor. Everyone tries to send out in a timely fashion but sometimes may be unable to send records at the last minute. Thank you.

Parking

Please advise that free parking in the Parking Structure 2 may be difficult at certain times
of the year. There is FREE valet parking in front of the Good Sam Hospital entrance. Download a map of building and parking.
