Below you will find helpful information about parking, insurance, appointment cancellation, and important things to bring with you for your appointment.
We ask that please bring these necessary items to your appointment:
Your Current Insurance Card(s) and photo ID:
We must have current information before you are seen. If you arrive without a current insurance card, we will need to reschedule your appointment. (You may take a picture of your card front and back and if active in MyHealth, attach within your portal-- or you may email or fax to us.
Your Co-pay:
We accept cash, check, money order, debit and credit cards Visa, Master Card, American Express and Discover cards.
Completed Health History Paperwork:*
All new patients : Before your appointment,
scheduled for either in-office or via telehealth, we ask that you
please complete and return all paperwork.

Established patients scheduled for your first telehealth visit: For all telehealth visits, insurance requires signed consent. Please download the telehealth consent form here, sign and either scan/email or fax. If your appointment is via telehealth, a medical assistant will call you approximately 10 minutes before your appointment to go over your health history. Thank you!
Established patients with a telehealth appointment, please also return to us the
HIPAA Acknowledgement and Consent
and our
Credit Policy.

Our providers are contracted with most major insurance companies. However, insurance policies and guidelines are always changing and vary from plan to plan. We do our best to verify your eligibility before your visit but sometimes we are unable to gather that information or are given incomplete/inaccurate information. If the provider is not in-network under your plan, you may be responsible for payment of a significant portion, if not all, of your visit’s charges. To avoid this situation, before your visit-- we ask that you please check with your insurance company to be sure:
Please note: For outside diagnostic services (such as labs and ultrasounds), we suggest you check with your insurance carrier regarding benefits and in-network facilities. When speaking with insurance representatives, you may need coding information which we are happy to provide.
If, for any reason, you are unable to keep your appointment and need to reschedule, please notify us. We appreciate 24 hours notice. We understand that unexpected things come up.
If you do not show up for your appointment and did not call beforehand to cancel —after two No Show appointments, we will charge your account $50.
A signed record release is necessary for release of your medical records from our office to another doctor’s office, a hospital or to you. Please download, complete, sign, date and return to us by fax or mail.
If you are being referred by another provider to our office it is important that your records from their office be available for the provider to review before your scheduled visit. Please allow at least a couple of days to get them from the referring doctor. Everyone tries to send out in a timely fashion but sometimes may be unable to send records at the last minute. Thank you.
Free parking in the Parking Structure 2 may be difficult at certain times of the year–the Winter holiday season especially. You will need a validation sticker to exit the parking garage at the end of your appointment. Please ask us to validate your ticket before you leave.
If you are having trouble finding a spot, please know there is FREE valet parking in front of the Good Sam Hospital entrance.
